This procedure applies to direct employees, subcontractors, agency workers and visitors. The Site Administrator owns the access register and the Site Manager approves exceptions recorded under this procedure. Before a worker’s first shiftThe subcontractor sends the worker roster by 16:00 on the previous working day. A late submission moves the worker to the next available induction slot. The Site Administrator verifies the worker’s identity and checks Safe Pass and task-specific CSCS evidence where applicable to the role. The access register records the result and the expiry date where one applies. The worker completes the Loughview induction and signs the attendance record before entering a work zone. An induction completed at another Corran Civil project cannot be used for Loughview access. The gate attendant activates the badge only after the register shows the worker as cleared. A badge identifies the person and does not grant permission for specialist work.Access decisionsPersonEvidenceAccess decisionSubcontractor electricianRole checks and site induction completedAccess to assigned zonesOffice-only visitorSigned visitor register and named escortReception and designated office route onlyDelivery driver unloading in a work zoneDelivery briefing and marshal assignedAccess to designated unloading area onlyExpired evidence and return to siteThe gate attendant blocks work-zone access when required evidence expires. The Site Administrator rechecks the replacement evidence before restoring access. Workers absent from Loughview for 60 calendar days or more repeat the site induction before their next shift.Example. A supplier attending a meeting can use the office-only visitor route with an escort. If the same supplier needs to inspect installed equipment in a work zone, the office-only exception no longer applies.Related procedure CCL-PTW-205 · Permit-controlled workFICTIONAL TEST DATA · CCL-PTW-205 · Revision 3 · Effective 9 November 2026Permit-controlled workThe Loughview permit register covers excavation, hot work and confined-space entry. The Permit Issuer reviews the proposed task and authorises the permit. The Performing Supervisor briefs the crew and closes out the work with the issuer. Each permit identifies the work area, scope, expiry time and named supervisor. When a permit is requiredActivityProject triggerEvidence attachedExcavationBreaking ground in a designated work zoneMarked work area and service-check recordHot workWelding or spark-producing cutting outside the designated workshopArea check and named fire-watch assignmentConfined-space entryEntry into an area listed in the site registerEntry assessment and rescue arrangementsIssue, pause and closeThe Performing Supervisor submits the permit request by 15:00 on the previous working day for planned work. The issuer checks the requested scope against other active permits in the same area. Before starting, the crew confirms the permit number and the work boundary during the task briefing. The register changes from Pending to Active only after the issuer signs.Stop and suspend the permit if conditions, scope or the named supervisor change. Only the Permit Issuer can return it to Active after reviewing the change and recording the decision. The permit expires at the recorded time or at the end of the shift, whichever comes first. The register changes from Pending to Active only after the issuer signs.Routine hot work in the designated workshop follows the workshop procedure. The exception applies only within the marked workshop boundary. An urgent job outside that boundary can skip the advance submission deadline with the issuer’s approval, but it still requires a signed permit before work begins.Example. Permit LV-HW-044 expires at 16:30. If work is still in progress at 16:30 and the shift ends at 17:00, the permit expires at 16:30.Related procedure CCL-INC-310 · Incident reportingFICTIONAL TEST DATA · CCL-INC-310 · Revision 2 · Effective 2 November 2026Incident and near-miss reportingThis procedure describes the fictional Loughview project’s internal reporting workflow. It covers injuries, near misses, damage and environmental events involving employees, subcontractors, delivery personnel and visitors. Internal deadlines below are project rules. The project safety lead assesses any external reporting requirements separately.Internal reporting deadlinesEventFirst notificationWritten recordInjury or high-potential near missNotify the Site Manager immediatelyWithin 2 hours of the eventOther near missNotify the crew supervisor before leaving siteBy the end of the same shiftProperty damage onlyNotify the crew supervisorWithin 24 hours of the eventWhat the register capturesRecord the occurrence time, report time, location, event type, immediate actions and the person assigned to follow up. Use the local site time and include the date when a shift crosses midnight. Describe what happened in plain language and record supporting photographs in the restricted incident record. If the person involved cannot submit the entry, the supervisor records it on their behalf. If the reporting system is unavailable, the supervisor uses the paper form and uploads it when access returns. The record preserves the original reporting time and adds a separate upload timestamp.Corrective action and close-outThe Site Manager assigns a corrective-action owner within one working day of receiving the written record. The project safety lead verifies the evidence before closing an action. An action with an overdue due date remains open in the weekly review until it is verified and closed.Example. A falling item narrowly misses a worker at 10:20. The supervisor classifies it as a high-potential near miss, notifies the Site Manager immediately and submits the written record by 12:20.Version note. Revision 1 allowed four hours for the written record after a high-potential near miss. Revision 2 supersedes it and requires two hours.Corran Civil Ltd · Loughview Business Park · Fictional test fixture
